Impotence : Symptoms, Cause and Treatment

Impotence or erectile dysfunction is the inability to achieve or maintain an erection to undergo sexual activity. Erection can occur because of the entry of blood flow to the penis.

This usually occurs after sexual stimulation is delivered to the brain, then passed to the innervation of the penis. However, there are some situations where erections cannot occur, such as in diabetes, and heart problems.


Diagnosis of erectile dysfunction can be made based on patient complaints. But the causes of erectile dysfunction can be sought by doing a number of examinations such as:

  • Hormone examination. A person with erectile dysfunction can undergo hormone testing to find out the secondary causes of this disorder. Examination of testosterone and LH (Luteinizing Hormone) can be done to see hormone disorders that can be the basis of erectile dysfunction disorders.
  • Duplex ultrasound examination can be done to check the blood flow to the patient's penis. In blood vessel disorders such as atherosclerosis, the blood flow to the penis will decrease and cause erectile disorders.
  • Penile skin sensitivity test. Penile sensitivity can be examined by checking skin sensitivity to vibration stimulation. Erectile dysfunction can occur because of an increase in the threshold of vibration of a person so that it is difficult to be aroused. This examination can detect whether there is a sensory disorder that underlies erectile dysfunction.


Erectile dysfunction is characterized by an inability to erect that occurs regularly and repeatedly. Despite sexual stimulation, penile erection cannot occur.

A person with erectile dysfunction usually has a risk factor for this disorder. Various diseases associated with erectile dysfunction are diabetes mellitus, high blood pressure and heart disease.


Handling of erectile dysfunction depends on the cause. Exercise is effective in preventing erectile dysfunction. For smokers, stopping smoking is one way to reduce the possibility of erectile dysfunction.

Various treatments can be tried to treat erectile dysfunction. Phosphodiesterase inhibitors are one type of drug that is widely available on the market to increase blood flow to the penis. This drug works by increasing the caliber of penile blood vessels.

Some examples of this class of drugs are sildenafil, vardenafil and tadalafil. Treatment in the form of ointment on the skin is also available. Alprostadil drug injection can also be done to treat erectile disorders. Injections can be done just before sexual intercourse.


There are many causes of erectile disorders. Therefore, searching for the right causes sometimes requires careful examination. The following are various causes of erectile dysfunction that need to be considered:

  • Drugs (antidepressants) and nicotine
  • Disorders of the cavernosum (peyronie's disease)
  • Psychological disorders, anxiety, stress and mental disorders
  • Surgery
  • Aging
  • Kidney disorders
  • Diabetes mellitus
  • Lifestyle, like smoking

Surgical complications are one of the causes of erectile dysfunction. Various surgeries that interfere with the nerves in the penis area can cause erectile dysfunction, for example the surgical removal of the prostate.

Erectile dysfunction can also be caused by metabolic diseases such as diabetes mellitus. In this disease, blood vessel damage and autonomic nerves occur. Disturbance of blood vessels can cause difficulty in the flow of blood that can enter the penis.
